Change Address
Make updates or changes to your account address:
 
  1. Online, 24 hours a day, using Shareholder Account Access.
  2. Contact a customer service representative (1-800-245-4770, Option 3).
  3. Complete the Change of Address Form and mail to the address listed on the form.

Please Note: Change of address forms can be found on the back of your statement.

Change Ownership of Account
Change of Ownership for Individual & Joint Tenant with Right of Survivorship Registration:
 
  • Add or remove an account owner. Complete the Change of Ownership Form. Please make sure that all account owners sign the form. You will receive an account statement with your new account number confirming the change.
  • Change account ownership because one account owner is deceased. Complete the Change of Ownership Form and include an original, certified death certificate. Please make sure that all surviving account owners sign the form. You will receive an account statement with your new account number confirming the change.
  • Change account registration to a trust registration. Complete the Change of Ownership Form. Please make sure that all account owners sign the form. You will receive an account statement with your new account number confirming the change.
  • Add beneficiaries to a non-IRA account. Complete the Transfer on Death Registration Beneficiary Form. Please make sure that all account owners sign the form.
  • Change account ownership because all account owners are deceased. Executor or personal representative of the most recently deceased account owner must complete the Change of Ownership Form. The following documents must also be provided:
    • Certified death certificate of all account owners except the most recently deceased
    • Certified copies of Letters of Testamentary or Order of Appointment of the most recently deceased account owner, dated within 60 days of the proposed transaction. (If date of death is not shown on this document, a copy of the last descendant's death certificate is needed.)
    • Some states require a Tax Waiver; if your state requires this waiver, you need to provide an original State Inheritance Tax Waiver. Please contact the State Department of Revenue for information about this requirement.
Change of Beneficiary Information

Complete the Individual Retirement Account (IRA) Beneficiary Form and mail to the address on the form.

Change your name on your account by mailing a letter of instruction to:

Your written instructions should include:

A signature guarantee is designed to protect your account from fraud. Obtain a signature guarantee from a bank or trust company, savings association, credit union or broker, dealer, or securities exchange member. A notary public cannot provide a signature guarantee.

Add Power of Attorney to Account
As Power of Attorney, you may:
 
  1. Change the registration, address, social security number, or distribution options;
  2. Purchase, redeem, or exchange shares;
  3. Direct dividend checks to an address other than the one of record;
  4. Establish some redemption services; and
  5. Request specific account information.

Complete the Indemnification Agreement for Power of Attorney Form and mail to the address listed on the form.

Please be sure to include notarized signatures of the account owner and the attorney or agent.
